Verification Form
Verification Worksheet

The University of Dayton's Office of Financial Aid is required by law to collect financial verification documents for those FAFSA filers who have been selected for review by the U.S. Department of Education. Traditionally about one-third of all FAFSAs filed are selected for verification.  If the Office of Financial Aid has not requested this information, please do not complete this process.

In addition to the verification worksheet, you must submit signed copies of all tax returns used to complete your FAFSA (including those of the student, the student's parent(s) (if dependent) and/or the student's spouse (if independent) to the Office of Financial Aid within thirty days of the original request.  The Office of Financial Aid is required to revise a student's financial aid eligibility based on any changes/corrections resulting from the review of the submitted documentation.

This form may be accessed using Adobe Acrobat Reader.  If you do not have this program on your computer, you may download this program free from the Adobe website by clicking onGet Acrobat Reader

 

By opening the reader within your browser, you will be able to complete the form on your computer, then print, sign and submit the form, along with signed copies of all tax returns, to our office.   Remember to keep a copy for your records before mailing or faxing the original to the Office of Financial Aid.  If you would like to save a copy to your local computer, right-click on the link below and select the "Save As" option.
