For students and families who have been selected for review, the Department of Education requires that the University of Dayton collect documentation to verify the information you reported on the Free Application for Federal Student Aid (FAFSA).  If the Office of Financial Aid has not requested this information, do not complete this form.

 

Instructions:
This form may be accessed using Adobe Acrobat Reader.  This option will allow you to complete the form on your computer, then print and sign the form before submitting it.  Once you have entered your data in the form, simply select print, then sign the printed copy and mail or fax the form to our office.
