Consortium Agreement - 2002-2003
Consortium Agreement for Study Abroad

For most students interested in participating in a study abroad program, a contract needs to be established between the University of Dayton and the host institution to receive financial aid to help cover the costs of the program.  The consortium agreement is the document that establishes the contract between the two schools.  For more information about using financial aid for your study abroad program, please click here.
 

Instructions:
This form may be accessed using Adobe Acrobat Reader.  This option will allow you to complete the form on your computer, then print and sign the form before submitting it.  Once you have entered your data in the form, simply select print, sign the printed copy and submit the form to the financial aid office at your host institution.  This form may be faxed or mailed to your host institution.  Once your host institution has completed the necessary information, the form should be returned to the Office of Financial Aid.

After the consortium agreement has been returned to the Office of Financial Aid, we will use the information provided by your host institution to determine the types and amount of financial aid you may be eligible to receive.  Your aid eligibility will then be confirmed with your host institution and a copy of this agreement will be mailed to you.  Funds will not be released until the consortium agreement has been completed in its entirety. 

If you have submitted a consortium agreement but have not yet received the completed copy listing your aid eligibility, please contact the Office of Financial Aid to speak to a Financial Aid Counselor to discuss the status of your consortium agreement.
