Statement of Certification for PLUS

If you seek to borrow under the Parent Loan for Undergraduate Students (PLUS) but will not be filing the Free Application for Federal Student Aid (FAFSA), the federal government requires that you complete a Statement of Certification regarding the intended use of educational loan funds.

If we receive a PLUS application for a student who has not filed the FAFSA, we will contact you to request that this certification be completed.

Instructions:

This form may be accessed using Adobe Acrobat Reader. This option will allow you to complete the form on your computer, then print and sign the form before submitting it. Once you have entered your data in the form, select print, sign the printed copy and mail or fax the form to our office.
